O'tkir miokard infarktidan keyin bemorlarda dori terapiyasiga rioya qilish: adabiyotlarni tizimli ravishda ko'rib chiqish
Annotatsiya
Maqsad. Ambulatoriya kuzatuvi paytida o'tkir miokard infarktidan keyin bemorlarda dori-darmonlarga rioya qilishni baholash va yaxshilashning mavjud yondashuvlarini tizimli ravishda tahlil qilish, ayniqsa davolanishga rioya qilishga ta'sir qiluvchi omillar va ko'p tarmoqli yondashuvning roliga e'tibor qaratish.
Materiallar va usullar. O'tkir miokard infarktidan keyin dori-darmonlarga rioya qilish bo'yicha adabiyotlarni tizimli ravishda ko'rib chiqish o'tkazildi. Yirik xalqaro tadqiqotlar ma'lumotlari, jumladan, 106 002 bemor ishtirok etgan 10 ta tadqiqotning meta-tahlili, 2023-yilgi Amerika Yurak Assotsiatsiyasining ilmiy bayonoti, 2023-yilgi ESC va 2025-yilgi ACC/AHA ko'rsatmalari va hozirgi yurak reabilitatsiya dasturlari tahlil qilindi.
Natijalar. Meta-tahlil shuni ko'rsatdiki, dori terapiyasiga yaxshi rioya qilish barcha sabablarga ko'ra o'limni (ehtimollik nisbati 0,56; 95% CI: 0,45-0,69), yurak-qon tomir o'limini (OR 0,66; 95% CI: 0,51-0,87) va kasalxonaga yotqizish darajasini (OR 0,61; 95% CI: 0,45-0,82) kamaytirish bilan bog'liq. Miokard infarktidan keyin bemorlarning 30 foizida terapiyaga rioya qilmaslik kuzatiladi. Birgalikda qaror qabul qilish, tuzilgan yurak reabilitatsiya dasturlari va ko'p tarmoqli guruhlarning ishtiroki uzoq muddatli davolanishga rioya qilishni sezilarli darajada yaxshilaydi.
Xulosa. Dori terapiyasiga rioya qilish o'tkir miokard infarktidan keyin bemorlarning prognozini yaxshilashda asosiy omil hisoblanadi. Birgalikda qaror qabul qilish, bemorlarni o'qitish, farmatsevtika bo'yicha maslahat berish va raqamli texnologiyalardan foydalanishni o'z ichiga olgan ko'p tarmoqli yondashuv rioya qilishni yaxshilashda eng katta samaradorlikni ko'rsatadi.
