Применение агонистов рецепторов ГПП-1 при кардиоренометаболическом синдроме

Аннотация
Кардиоренометаболический синдром (КРМС) является одной из наиболее значимых проблем современной медицины и относится к ведущим причинам заболеваемости и смертности во всем мире. Крупные рандомизированные клинические исследования последних пяти лет — SELECT (2023), FLOW (2024) и STEP-HFpEF (2023) — предоставили принципиально новые данные о кардио- и ренопротективных механизмах агонистов рецепторов глюкагоноподобного пептида-1 (ГПП-1), в частности семаглутида. Эффекты агонистов рецепторов ГПП-1 выходят за рамки гликемического контроля и охватывают многокомпонентную биологическую систему, направленную на коррекцию эндотелиальной дисфункции, окислительного стресса, воспаления, опосредованного NF-κB, активации ренин-ангиотензин-альдостероновой системы (РААС), внутриклубочковой гиперфильтрации и висцерального ожирения. Имеющиеся клинические и механистические данные дают достаточные основания рассматривать данный класс препаратов как один из ключевых компонентов будущей стратегии ведения пациентов с кардиоренометаболическим синдромом.
