Annotatsiya

Aortokoronar shuntlashdan (AKSh) keyin surunkali yurak yetishmovchiligining (SYuY) avj olishi kardiojarrohlikning dolzarb muammosi bo‘lib qolmoqda. Sharhda angiotenzin II ning birinchi tur retseptori geni (AGTR1) A1166C (rs5186) bir nukleotidli polimorfizmining infarktdan keyingi remodellanish va jarrohlik revaskulyarizatsiyasi natijalariga ta’siri haqidagi zamonaviy ma’lumotlar tizimlashtirilgan. Adeninning sitozinga almashinuvi mikroRNK-155 ning gen mRNKsi bilan bog‘lanishini buzib, hujayra membranalarida AT1R retseptorlarining patologik giperekspressiyasiga olib keladigan molekulyar-genetik mexanizm ko‘rib chiqilgan. Jarrohlik stressi sharoitida mutant C allelini tashuvchilik tizimli yallig‘lanish, endotelial disfunksiya, miokard fibrozining tezlashishi, shuntlar trombozi xavfining oshishi va bo‘lmachalar fibrillyatsiyasining rivojlanishi bilan bog‘liq. Farmakogenetik jihatga alohida e’tibor qaratilgan: C alleli tashuvchilarida AAF ingibitorlari bilan davolashda «ta’sirdan chiqish» fenomeni tavsiflangan bo‘lib, bu angiotenzin retseptorlari blokatorlarini (ARB/sartanlar) ustuvor buyurishni patogenetik jihatdan asoslaydi. Neyrogumoral blokadani individuallashtirish va bemorlarning uzoq muddatli prognozini yaxshilash uchun rs5186 lokusini genotiplashning klinik ahamiyati ta’kidlangan.
