Clinical case of pseudo-Wellens syndrome caused by pulmonary embolism
Abstract
Pseudo-Wellens' syndrome is a rare clinical condition that mimics Wellens' syndrome. This article presents a case of pseudo-Wellens syndrome associated with pulmonary embolism (PE). A female patient was
admitted to the emergency room one hour post-syncope, experiencing recurrent episodes of loss of consciousness and shortness of breath. Examination revealed elevated levels of C-reactive protein, D-dimer, interleukin-6 (IL-6), and N-terminal pro-brain natriuretic peptide (NT-proBNP). Electrocardiography (ECG) showed T-wave inversions in leads V1-V6. Coronary angiography ruled out significant stenosis, while computed tomography pulmonary angiography (CTPA) confirmed acute PE. The patient underwent fibrinolysis with 50 mg of tenecteplase. Upon discharge, the patient was recommended to take warfarin at a dose of 7.5 mg/day as anticoagulant therapy.
