Annotatsiya

Tadqiqot maqsadi: arterial gipertenziyali (AG) bemorlarda telmisartan/amlodipin fiksirlangan kombinatsiyasi bilan 6 oylik terapiyaning antigipertenziv samaradorligini, kardiometabolik xavf omillariga ta'sirini, nishon organlar zararlanishining regressini va xavfsizligini baholash.


Material va usullar. Tadqiqotga ESH 2023 yil tasnifiga ko'ra I–II darajali AG bilan og'rigan 18–65 yoshli (o'rtacha yosh 51.90±13.84) 52 nafar bemor jalb qilindi. Barcha bemorlarga telmisartan/amlodipin fiksirlangan kombinatsiyasi tayinlandi. Tekshiruv boshlang'ich holatda va 6 oydan keyin o'tkazildi: ofisdagi AB (arterial bosim) o'lchandi, exokardiografiya, applanatsion tonometriya, uyqu arteriyalarining dupleks skanerlashi o'tkazildi, glyukoza, lipidlar, siydik kislotasi, jigar transaminazalari, kreatinin darajalari koptokchali filtratsiya tezligini (KFT) hisoblash bilan birga baholandi, ertalabki siydikda mikroalbuminuriya (MAU) aniqlandi.


Natijalar. Telmisartan/amlodipin fiksirlangan kombinatsiyasi bilan 6 oylik terapiyadan so'ng SAB (sistolik arterial bosim) 19.54% ga, DAB (diastolik arterial bosim) 16.67% ga pasaydi (p<0,001). Chap qorincha gipertrofiyasining (ChQG) sezilarli regressi qayd etildi, bu ChQMI (chap qorincha miokardi massasi indeksi)ning 7.6% ga kamayishi bilan baholandi. PTTT (puls to'lqinining tarqalish tezligi) statistik jihatdan ahamiyatli darajada 9.95±2.13 m/s dan 9.00±1.12 m/s gacha pasaydi (p=0.019). KFT oshdi. O'tkazilayotgan terapiya fonida lipid profili yaxshilandi (UX (umumiy xolesterin) darajasi (p=0.009) va PZLP-X (past zichlikdagi lipoproteidlar xolesterini) (p=0.018) pasaydi).


Xulosa. Telmisartan/amlodipin bilan fiksirlangan kombinatsiyali olti oylik terapiya yuqori antigipertenziv samaradorlikni, aniq kardio-, vazo- va nefroprotektsiyani, shuningdek AG bemorlarida metabolik profilga ijobiy ta'sirni namoyish etdi, bu uni, ayniqsa kardiometabolik xavf omillari mavjud bo'lganda, tanlov kombinatsiyasiga aylantiradi.
