Annotatsiya

 


Maqsad. Ishemik kardiomiopatiya (IKMP) bilan kasallangan bemorlarda 6 oylik davolanish mobaynida bemor hayot sifati (KCCQ-12) qay darajada tiklanishini, koronar revaskulyarizatsiya to’liqligiga bog’liqligi, hamda uni belgilovchi mustaqil omillarni aniqlash.


Materiallar va usullar. Prospektiv bir markazli tadqiqotga IKMP bilan kasallangan 168 bemor kiritildi. Yurak jamoasi (Heart Team) bemorlarni uch guruhga ajratdi: 1-guruh – to’liq teri orqali koronar aralashuv (TOKA) (n=58), 2-guruh – to’liqsiz TOKA (n=54), 3-guruh – faqat OMT (n=56); barcha bemorlar SGLT2 ingibitori qabul qildi. Uzluksiz natijalar boshlang’ich qiymat hamda qoldiq SYNTAX ballga tuzatilgan ANCOVA yordamida, javob bashoratchilari logistik regressiya bilan tahlil qilindi.


Natijalar. 6 oyda KCCQ-12 barcha guruhlarda ishonchli ravishda oshdi (1-guruh +14,3; 2-guruh +9,5; 3-guruh +9,5 ball; guruh ichida barcha p<0,05); ≥5 ballga klinik ahamiyatli yaxshilanishga bemorlarning 53,7–63,8%i erishdi. Guruhlararo tuzatilgan KCCQ farqlari ahamiyatsiz bo’ldi (1- va 3-guruh: +3,28 ball; 95% CI −12,8–19,3; p=0,69). To’liq revaskulyarizatsiya qo’shimcha CHQOF o’sishi bilan bog’landi (1- va 3-guruh: +5,01%; 95% CI 1,17–8,84; p=0,01). KCCQ o’zgarishi CHQOF o’zgarishi bilan zaif, ammo ishonchli korrelyatsiyada edi (Spearman r=0,158; p=0,040). ≥10 ballga javobning yagona mustaqil bashoratchisi – pastroq boshlang’ich KCCQ (har bir ball uchun OR 0,91; 95% CI 0,88–0,94; p<0,001); revaskulyarizatsiya to’liqligi mustaqil bog’lanmadi.


Xulosa. SGLT2 ingibitori asosidagi kvadroterapiya fonida IKMP bilan kasallangan bemorlarda hayot sifati revaskulyarizatsiya to’liqligidan qat’i nazar sezilarli va o’xshash darajada yaxshilandi, to’liq revaskulyarizatsiya esa qo’shimcha chap qorincha teskari remodellanishini ta’minladi. Eng katta hayot sifati foydasi dastlab eng ko’p simptomga ega bemorlarda kuzatilgan bo’lib, bu shaxsiylashtirilgan, bemorga yo’naltirilgan qaror qabul qilishni qo’llab-quvvatlaydi.
