Yurak ishemik kasalligining avj olishida yallig‘lanish,  epigenetik va kardiovizualizatsiya biomarkerlarni kompleks baholash

Annotatsiya

Maqsad. Ushbu tadqiqotda epigenetik biomarkerlar (miR-155), yallig‘lanish sitokinlari (TNF-α, IL-1β) va noinvaziv kardiovizualizatsion andozalarni o‘z ichiga olgan multibiomarker modelining ishemik yurak kasalligi (IYuK)ning progres-slashuvchi kechishini baholashdagi prognoz qiymati o‘rganildi.


Materiallar va usullar. Barqaror va beqaror shakldagi IYuK tashxisi qo‘yilgan 152 nafar bemorda kontrastli MSKT-angiografiya natijalariga asoslangan koronar ateroskleroz parametrlari (koronar kaltsiy indeksi, stenoz darajasi, remodellashtirish indeksi, jalb etilish indeksi – SIS, blyashkalar zichligi, hajmi va massasi) bilan biomarkerlar darajasi o‘rtasidagi bog‘liqlik kompleks baholandi. miR-155 ekspressiyasi real vaqt rejimida polimeraz zanjir reaktsiyasi (PZR) usuli yordamida Rotor-Gene Q qurilmasida aniqlandi, yallig‘lanishga qarshi sitokinlar konsentratsiyasi esa immunoferment analiz (IFA) usuli bilan o‘lchandi.


Natijalar. IYuK bo‘lgan bemorlarda mikroRNK-155 giperekspressiyasi va yallig‘lanish sitokinlarining konsentratsiyasi oshganligi prognoz nuqtai nazaridan muhim ahamiyatga ega ekani tasdiqlandi. miR-155 darajasi bilan IL-1 (r=0,71; p<0,01) va TNF-α (r=0,78; p<0,01) o‘rtasida ijobiy va mustahkam korrelyatsiyalar kuzatildi. Shuningdek, miR-155 ekspressiyasi blyashkalar hajmi, soni va ularning past zichlikdagi, zaifroq va yuqori xavfli shakllari bilan sezilarli bog‘liqlikka ega ekani aniqlandi.


Xulosa. Olingan ma’lumotlar ushbu multibiomarker paneli IYuK ni erta aniqlash, xavf darajasini baholash va individual yondashuv asosida davolash strategiyalarini ishlab chiqishda istiqbolli vosita bo‘lib xizmat qilishini ko‘rsatadi. Epigenetik, yallig‘lanish va kardiovizualizatsion biomarkerlarni birgalikda baholash xavfni aniqlash aniqligini oshirishi va klinik qarorlar qabul qilish jarayonini yaxshilashi mumkin.
