Abstract

Aim of the study: To evaluate the diagnostic significance of ABPM and to identify the features of the daily blood pressure profile (BPAP) in patients with hypertension in combination with type 2 diabetes.

Materials and methods of research: 412 patients with AH I-III degree and type 2 diabetes were analyzed. All patients underwent clinical and instrumental studies. The 24-hour blood pressure profile  was assessed using the “Medicom-combi” apparatus  (Russia) with  the oscillometric method. Statistical analysis was carried out using the SPSS 27.0 application package.

Results of the study: Patients were divided into 2 groups taking into account the presence of diabetes: group 1 consisted of patients with AH and diabetes 2 (n=121), group 2 patients with AH without diabetes 2 (n=291). When analyzing ABPM indicators, it was revealed that hypertensive patients with type 2 diabetes had statistically significant high values of average daily SBP (p = 0.023) and DBP (p = 0.021), average daily SBP (p = 0.046), average daily PBP (p = 0.012 ) compared to hypertensive patients without diabetes. The variability of SBP and DBP between groups did not differ significantly. In patients with hypertension and type 2 diabetes, there was a statistically significant insufficient decrease in SBP at night (p = 0.019). Analysis of dipping status showed that in patients with hypertension and type 2 diabetes, the number of dippers according to SBP was significantly lower compared to patients with hypertension without type 2 diabetes (p = 0.0004), the number of non-dippers and night-pickers was also greater in the group of patients with hypertension and type 2 diabetes, but without statistical significance. The groups did not differ from each other in the number of overdippers.

Conclusions: To identify the characteristics of ABPM, it is advisable for all patients with hypertension and type 2 diabetes to undergo ABPM. Patients with hypertension and type 2 diabetes are characterized by more pronounced disturbances in SPBP, in the form of a lack of adequate reduction in nocturnal blood pressure.
